If the text "FORMTEXT" appears in the fields of the form, you'll have to change the settings in your Word client. To do this, follow the instructions below:
1. Create a new document using Word.
2. Enter "Tools", "Alternatives" and then the "View" tab.
3. Uncheck "Field codes" and click Ok.
4. Choose "Save as" in the File menu, name the document and save.
